There are two types of benzodiazepine receptors. The central type is associated with the GABAa receptor complex and mediates the anxiolytic and anti-convulsive properties of benzodiazepines (e.g., diazepam, also known as valium) in the central nervous system.24 Central type benzodiazepine receptors are known to be present in high concentrations in the brain, but not in peripheral organs. Tranquilizers and antiinflammatory drugs

Drugs administered to food animals to treat acute conditions include tranquilizers and anti-inflammatory drugs (Figure 3). These medicines might be given to animals shortly before they are sent to slaughter, resulting in potentially high levels of residues that may persist in edible tissue. For example, tranquilizers, such as acepromazine, azaperone, chlorpromazine, propionlypromazine and xylazine, have been shown to reduce stress or aggressiveness during transport for certain species of pigs 13 . The b-blocker carazolol has also been used 14 . In the U.S., azaperone is approved for the control of aggressiveness in small pigs. In the European Union (EU), maximum residue limits have been set for carazolol and azaperon the use of chlorpromazine in food animals is not allowed 13 .

Glial proteins as markers of neurotoxicity

We have focused our efforts on the peripheral benzodiazepine receptor (PBR), a glial specific protein. The novel aspect of the PBR approach is that it can overcome many of the limitations of presently available methods, and has the added advantage of being able to be used for in vivo assessment of neurotoxicity. The latter is possible because of the availability of a highly specific and selective ligand that can be labeled with a variety of radioisotopes for in vitro as well as in vivo imaging studies (See Positron Emission Tomography of Peripheral Benzodiazepine Receptor Expression in Human Neurological Disease below). Therefore, the PBR method offers for the first time a noninvasive, quantitative, high resolution, imaging approach to assess neurotoxic endpoints resulting from chemical-induced or other types of brain damage.

Cancer

A worker who splattered pure 2,4-DCP on portions of his right arm and leg while disposing of industrial waste collapsed and experienced a seizure within 20 minutes of the accident and died shortly thereafter. Postmortem examination revealed blood and urine 2,4-DCP concentrations of 24.3 and 5.3 mg L, respectively. The identity of 2,4-DCP was confirmed by mass spectrometry. The investigators did not estimate an absorbed dose (Kintz et al. 1992), but assuming a blood volume of 5 liters and a body weight of 70 kg, the dose would be approximately 2 mg kg as a minimum. A screen for other drugs including ethanol, organic solvents, tranquilizers, and drugs of abuse was negative.

Acute ingestion

A 46 year old male presented to hospital 30 minutes after ingestion of about 1 quart (940 ml) of a toluene containing paint thinner. He complained of abdominal pain and developed vomiting and diarrhoea. On examination he was alert but mildly ataxic and became disorientated. He had a non-anionic gap metabolic acidosis. Toxicological analysis of serum was positive for benzodiazepines, opioids and tetrahydrocannabinnol (cannabis). His CNS depression worsened and he was electively intubated to protect the airway during gastric lavage. Gastric contents were bloody. He recovered within 36 hours with supportive treatment (Caravati and Bjerk, 1997).

We collected data on the date of admission, age, sex, estimated amount of GlySH ingested, co-ingestants of other agrochemicals, ethanol, or pharmaceuticals, suicide attempts, out-of hospital interval, initial clinical presentation, initial laboratory data in the ED, and clinical course. The EU conducts an extensive residue-monitoring program the results from the 2004 program have been posted 211 . This report includes residue-monitoring data from 25 countries (member states). In 2004, approximately 807,000 targeted samples and 64,000 suspect samples were tested. In addition to antibiotics testing, samples were also monitored for hormones and other veterinary drugs. Many types of food matrices were part of this program, including milk, eggs, honey, aquaculture and meat products. Residues of prohibited substances such as chloramphenicol and nitrofurans were found to be violative at a rate of approximately 0.11 . Residues of malachite green were found in aquaculture products from many of the member states. The violation rate for antibiotics was 0.22 . There were a few violative results for other drugs such as tranquilizers and NSAIDs and the incidence of coccidiostat residues in eggs was highlighted as a continuing problem.
